Required Documents and Forms
Worker
- Original valid U.S. passport/U.S. Birth Certificate/Naturalization Certificate.
- Form SSA-21 “Supplement for Claim of Person Outside the United Sates”
- Form SSA-1 BK “Application for Retirement benefits”
- Form SSA-795 “Statement of Claimant”
- Form SS-5 ''Application for a Social Security Card’’ if the full name on SSN Card is different from US passport.
- Form SSA-308 ‘’Modified Benefit Formula Questionnaire Foreign Pension’’ if the worker is receiving foreign pension, presenting evidence showing the amount and date of entitlement is mandatory.
- For lawful permenant residents (LPR), submission of valid Green Card (I-551) and passport is required (work records, if available).
Worker’s spouse
- Original valid U.S. passports of both worker and spouse.
- Original U.S. Birth Certificate or Naturalization Certificate or CRBA (Consular Report of Birth Abroad) of the applicants.
- Original U.S. Marriage record only if married in the U.S.
- Original Divorce Decree, if divorced.
- Form SSA-21 “Supplement for Claim of Person Outside the United States”
- Form SSA-2BK “Application For Wife’s or Husband’s Insurance Benefits”
- Form SSA-3 ‘'Marriage Certification’'
- Form SSA-7163 ''questionnaire about employment/self-employment outside the US’’ if currently working.
Worker’s child (below 16)
- Original valid U.S. passports of the child and the working parent.
- Original U.S. birth certificate or CRBA (Consular Report of Birth Abroad).
- Marriage certificate of the parents.
- Form SSA-4 “Applications for child Insurance Benefits”
- Form SSA-21 “Supplement for Claim of Person Outside the United Sates”
- Form SSA-1372FC ‘’Advanced Notice for Termination of Child’s Benefits’’ (age 17-19). The form should be completed and certified by school.
- Form SSA-11 if the child is a minor (available at our office)
Please note
- The personal presence of the applicant (s) is mandatory, minors have to be accompanied by at least one parent.
- Applicants can submit the mentioned documents, by appointment only. Appointments can be scheduled by phone or physical presence.
- Only complete applications can be accepted and further processed without any exceptions. Incomplete files will be refused and are subject to new appointment.
- Requested forms should be duly signed and dated during your visit at this office.
- Additional forms or documents may be required depending on your case.
